§ 45-49a-63-193

Each member or other person entitled to a benefit, before any benefit shall be payable to him or her or on his or her account under the plan, shall file with the board the information that it shall require to establish his or her rights and benefits under the plan.(Act 97-689, p. 1379, §11(10.04).
