04Submitted Ballot Questions.(a) At the election the question to be submitted shall beprinted in plain prominent type on separate ballots and shall read as follows: 'Shall the council-manager form of government, asprovided by the City Manager Act of 1977 be adopted for the City of PhenixCity? 'Yes . 'No .'(b) The voter shall mark his or her ballot with a crossmark before or after the word which expresses his or her choice. No other question shall be submitted tothe voters of the city upon this ballot. Ifvoting machines are used at any voting place in the election, the abovequestion may at the discretion of the election commission of the city or otherbody having charge of the conduct ofmunicipal elections in the city, be submitted as a separate question on votingmachines so used.
