The application for cancellation must be in the form and manner designated by the department and shall be signed on behalf of the foreign limited liability company by (1) a person with authority to sign the application under the law of the state or other jurisdiction of its organization; or (2) if the company is controlled by a receiver, trustee, or other court-appointed fiduciary, by the receiver, trustee, or other fiduciary.
