16-151. Forms for registration supplied by county recorder and secretary of state A. Blank state voter registration forms for registration of electors shall be supplied by the county recorder. B. The secretary of state shall make available for distribution through governmental and private entities the voter registration forms that are prescribed by the election assistance commission.
