Section 17-81-314 - Reinstatement

(a) A chiropractic license which has been revoked for repeated violation of any of the causes enumerated in § 17-81-313 shall not be eligible for reinstatement.(b)(1) A license which has been suspended may be reinstated upon expiration of the period of suspension and upon satisfactory assurance of proper conduct, by notarized statement of intent, by the suspended licensee.(2) The statement of intent shall be filed with the Secretary of the Arkansas State Board of Chiropractic Examiners at least thirty (30) days before the expiration of the period of suspension, accompanied by a fee of fifty dollars ($50.00) if the period of suspension is less than a year, and additionally by the regular yearly renewal fee if the period of suspension exceeds twelve (12) months.(3) All fees shall be paid by certified check or postal money order.Acts 1971, No. 706, § 27; A.S.A. 1947, § 72-441.
