(a) The following information shall accompany every policy of life insurance, accident and health insurance, property insurance, casualty insurance, or title insurance issued and covering risks located, resident, or to be performed in the State of Arkansas: (1) The complete address and telephone number, including a toll-free number if available, of the policyholder's service office of the company issuing the policy;(2) The name, address, and telephone number of the producer or agency soliciting the policy if applicable; and(3) The address and telephone number, including a toll-free number if available, of the State Insurance Department.(b) A person who fails to comply with this section is subject to the penalties provided in § 23-60-108.(c) The Insurance Commissioner may adopt appropriate rules to enforce and carry out the intent and purposes of this section.Acts 1987, No. 197, §§ 1-3; 2001, No. 1604, § 102; 2009, No. 726, § 38.
