17424. (a) A parent who has been served with a medical insurance form shall complete and return the form to the local child support agency’s office within 20 calendar days of the date the form was served.(b) The local child support agency shall send the completed medical insurance form to the department in the manner prescribed by the department.(Added by Stats. 1999, Ch. 478, Sec. 1.
