(a) The clerk of the court shall collect a fee of fifteen dollars ($15) per signature for any document that is required to be authenticated pursuant to court order.(b) Each document authenticated by the clerk of the court shall contain the following statement: '____, Clerk of the Superior Court, County of ____, State of California. Signed pursuant to court order dated ____ in the matter of ____ petitioner v. ____, respondent, Case No. ____.'(Added by renumbering Section 26835.1 by Stats. 2005, Ch. 75, Sec. 78.
