Colorado Legal Code
(1) Every person who desires to qualify for practice as a dental hygienist within this state must file with the board:(a) A written application for a license, on which application the applicant shall list: (I) Any act the commission of which would be grounds for disciplinary action under section 12-220-201 against a licensed dental hygienist; and(II) An explanation of the circumstances of the act; and(b) Satisfactory proof of graduation from a school of dental hygiene that, at the time of the applicant's graduation, was accredited, and proof that the program offered by the accredited school of dental hygiene was at least two academic years or the equivalent of two academic years.(2) The applicant must apply for a dental hygienist license on the form prescribed and furnished by the board, must verify the application by oath, and must submit the fee established pursuant to section 12-20-105 with the application.(3) An applicant for licensure who has not graduated from an accredited school or program of dental hygiene within the twelve months immediately preceding application, or who has not engaged either in the active clinical practice of dental hygiene or in teaching dental
