§ 3206

Each application for a license shall be made in writing and under oath to the Commissioner in such form and containing such information as the Commissioner may prescribe including the name and business and residence address: (1) Of the proprietor, if the applicant is an individual; (2) Of every member, if the applicant is a partnership or unincorporated association; (3) Of the corporation and each officer and director thereof, if the applicant is a corporation, stating the date and the state of incorporation.5 Del.
