§ 12-10-6-6

The application must be:(1) in writing or reduced to writing;(2) made in the manner and upon the form prescribed by the division; and(3) verified by the oath of the applicant.(b) If an individual applying for assistance is mentally or physically unable to sign an application verified by the oath of the applicant, an application may be made by an interested individual acting in the applicant's behalf, and the application shall be verified upon the basis of information and belief of the interested individual.(c) Each application must contain a statement of all of the following:(1) The amount of property, both personal and real, in which the applicant has an interest.(2) All income that the applicant has at the time of the filing of the application.(3) Any other information prescribed by the division.[Pre-1992 Revision Citation: 4-28-15-4.]As added by P.L.2-1992, SEC.4.
