When assistance is granted to a dependent child under IC 12-14-1 through IC 12-14-8, the award made must be entered on a written notice prescribed by the division that designates the following:(1) The name and residence of the recipient.(2) The amount of the award.(3) The date when the assistance is to begin.(4) Any other fact required by the division.[Pre-1992 Revision Citation: 12-1-7-8 part.]As added by P.L.2-1992, SEC.8.
