§ 14-24-9-1

(a) If, as a result of an inspection under this article, a written notice or order is issued for the treatment, removal, or destruction of a plant or element of beekeeping, the division may require that an affidavit be filed by the person to whom the notice or order was issued.(b) The affidavit described in subsection (a) shall be completed on a commission form and must specify the following:(1) That the terms of the notice or order have been satisfied to the best of the affiant's ability.(2) That the work was effective for the purpose prescribed.[Pre-1995 Recodification Citation: 14-7-9-1.]As added by P.L.1-1995, SEC.17.
