1 of this chapter is filed with a local health officer, the local health officer shall do the following:(1) Correct the local record of birth by adding the name of the father to the certificate of birth.(2) File a copy of the affidavit with the report required by IC 16-37-1-6.[Pre-1993 Recodification Citation: 16-1-16-16(b).]As added by P.L.2-1993, SEC.20.
