As used in this chapter, 'direct primary care agreement' means a contract entered into between a primary care provider or an employer of a primary care provider and an individual patient or the patient's legal representative in which the primary care provider or the employer of the primary care provider:(1) agrees to provide primary care health services to the individual patient for an agreed upon fee and time;(2) does not bill a third party that provides coverage to the patient for the primary care health services; and(3) charges a periodic fee for the primary care health services.As added by P.L.108-2017, SEC.1.
