5. Subject to IC 25-1-6.5, the committee must include the following:(1) At least two (2) occupational therapists who:(A) are residents of Indiana;(B) have at least three (3) years experience as occupational therapists; and(C) are licensed under this article.(2) At least one (1) physician licensed under IC 25-22.5 who is familiar with the practice of occupational therapy.(3) At least one (1) person who:(A) is a resident of Indiana; and(B) is not associated with occupational therapy in any way other than as a consumer.As added by P.L.238-1989, SEC.1.
