(2) A health maintenance organization that:(A) is organized under the laws of a state other than Indiana, a territory or another insular possession of the United States, or the District of Columbia; and(B) has obtained a certificate of authority under IC 27-13-2.(3) A limited service health maintenance organization that:(A) is organized under the laws of a state other than Indiana, a territory or another insular possession of the United States, or the District of Columbia; and(B) has obtained a certificate of authority under IC 27-13-34.(4) A fraternal benefit society that:(A) is organized under the laws of a state other than Indiana, a territory or another insular possession of the United States, or the District of Columbia; and(B) has obtained a certificate of authority under IC 27-11-8-5.As added by P.L.186-1996, SEC.1.
