The report must be:(1) made on forms prescribed by the commissioner; and(2) verified by at least two (2) principal officers of the limited service health maintenance organization.(b) In addition to the report required by subsection (a), a limited service health maintenance organization shall file with the commissioner on or before June 1 of each year an audited financial statement of the limited service health maintenance organization for the preceding calendar year.(c) The commissioner may require any additional reports necessary to enable the commissioner to carry out the duties of the commissioner under this chapter.As added by P.L.26-1994, SEC.25.
