Subject to sections 10 and 11 of this chapter:(1) a group or an individual contract;(2) an evidence of coverage; or(3) an amendment to:(A) a group or an individual contract; or(B) an evidence of coverage;may not be delivered or issued for delivery in Indiana unless the form has been filed with and approved by the commissioner.As added by P.L.26-1994, SEC.25.
