§ 27-8-12-2

As used in this chapter, 'certificate' means a document issued to a member of the group covered under a group insurance policy, which policy has been delivered or issued for delivery in Indiana, to signify that the individual named in the certificate is covered under the policy.As added by P.L.275-1987, SEC.1.
