The term includes the following:(1) A policyholder of an individual health insurance policy.(2) A member of the group covered by a group health insurance policy.(3) A female who is entitled to coverage under a health insurance policy as a spouse or dependent of an individual referred to in subdivision (1) or (2).As added by P.L.192-1996, SEC.2.
