A licensee shall give written notice to the department of the closing of an existing branch under subdivision (1) not later than thirty (30) days after the date of the closing of the branch. Not later than thirty (30) days before the intended relocation or addition of one (1) or more branches under subdivision (2), the licensee shall provide to the department the written notice of its intention required by this section and shall request approval in a form prescribed by the director to add or relocate one (1) or more branches.(b) A person that is licensed under this chapter, or a person that seeks a license or a renewal of a license under this chapter, in accordance with sections 4, 5, and 8 of this chapter, shall notify the department not later than thirty (30) days after any of the following occurs:(1) The person has a change in name, address, or principals.(2) The person files for bankruptcy or reorganization.(3) The person is notified by a state or governmental authority that the person is
