5(b) of this chapter.(b) If the application contains all the information required to be supplied by the voter, but does not include:(1) information required to be supplied by an individual who received the completed application from the voter; or(2) the signature of that individual;the county voter registration office shall promptly make one (1) effort to contact the individual who received the completed application from the voter, if possible, and one (1) effort to contact the voter to obtain the missing information or the signature in the case of the individual who received the completed application from the voter.
