As used in this chapter, 'collaborative care agreement' means a voluntary agreement that:(1) is signed by the department, a guardian ad litem or court appointed special advocate participating with the consent of the youth, and the older youth;(2) is approved by a juvenile court under this chapter;(3) includes provisions required or authorized under the department's rules concerning collaborative care services; and(4) may be amended by agreement between the department, a guardian ad litem or court appointed special advocate participating with the consent of the youth, and the older youth without review or approval by the court.As added by P.L.48-2012, SEC.34.
