5 and IC 31-34-21, means a foster parent who has provided care and supervision for a child:(1) for at least the twelve (12) most recent months;(2) for at least fifteen (15) months of the most recent twenty-two (22) months; or(3) if the child is less than twelve (12) months of age, for at least six (6) months.As added by P.L.152-2003, SEC.1.
