(b) As used in this section, 'state employee health plan' means one (1) of the following:(1) A self-insurance program established under section 7(b) of this chapter to provide group health coverage.(2) A contract with a prepaid health care delivery plan that is entered into or renewed under section 7(c) of this chapter.(c) A state employee health plan may provide coverage for methadone if the drug is prescribed for the treatment of pain or pain management as follows:(1) If the daily dosage is not more than sixty (60) milligrams.(2) If the daily dosage is more than sixty (60) milligrams, only if:(A) prior authorization is obtained; and(B) a determination of medical necessity has been shown by the provider.As added by P.L.209-2015, SEC.1.
