536C.4 Notification. 1. A person shall file a registration statement annually with the administrator before conducting the business of issuing credit cards in this state, and annually thereafter on orbefore January 31 of each year. The registration statement shall be in writing on a formprescribed by the administrator, and contain the name and address of the registrant, thename and address of a designated agent upon whom service of process may be made in thisstate, and any other information the administrator deems relevant. 2. At the time of filing a registration statement the person shall provide the administrator with a copy of the credit agreement and billing statement being used by the card issuer. 3.
