Any person who violates such permanent injunction or presents or attempts to file as the person’s own the certificate of licensure of another, or who shall give false or forged evidenceof any kind to the board, or to any member thereof, in obtaining a certificate of licensure, orwho shall falsely impersonate another practitioner of like or different name, or who shall useor attempt to use a revoked certificate of licensure, shall be deemed guilty of a fraudulentpractice.
