633.604 Application. The application for appointment of a foreign conservator or guardian as conservator in this state shall include the name and address of the nonresident ward, and of thenonresident conservator or guardian, and the name and address of the resident conservatorto be appointed. It shall be accompanied by a certified copy of the original letters or otherauthority conferring the power upon the foreign conservator or guardian to act as such. Theapplication shall also state the cause for the appointment of the foreign conservator to act assole conservator, if such be the case.
