286.9 -060 Materials to accompany application -- Investigation fee. Each application for a license shall be accompanied by: (1) An investigation fee of five hundred dollars ($500) for Kentucky residents and five hundred dollars ($500) for nonresidents of Kentucky for each location which shall not be subject to refund but w hich, if the license is granted, shall constitute the license fee for the first license year or part thereof; (2) Audited financial statements prescribed by the commissioner; and (3) Evidence that the applicant has complied or will comply with all workers' , and unemployment compensation laws of Kentucky.
