304.99 -125 Penalty for violation of KRS 304.14 -135. Any person violating KRS 304.14 -135 shall be fined not less than one hundred dollars ($100) for each offense. Each claim that is required by the insurer to be submitted on a form which is found in violation of KRS 304.14 -135 shall constitute a separate off ense.
