It shall be the duty of the Commonwealth's attorney after the imposition of prison sentence, unless the defendant is grante d probation, to cause to be forthwith transmitted to the institution with the commitment papers, a concise statement of the facts adduced at the trial or at the hearing of a plea of guilty, together with such notes of testimony as the board may request.
