RS 12:1-121 - Forms A.(1) The secretary of state may prescribe and furnish on request forms for any of the following: (a) An application for a certificate of existence and standing. (b) A foreign corporation's application for a certificate of authority to do business in this state. (c) A foreign corporation's application for a certificate of withdrawal. (d) The annual report. (2) If the secretary of state so requires, use of these forms is mandatory. B.
