RS 22:1524 - Notice required; application; fee A. No lessor shall issue or offer to issue a collision damage waiver in Louisiana until a notice has been filed with the commissioner of insurance as provided in this Part. Notice shall be filed in writing, in a form prescribed by the commissioner, and shall be accompanied by an application fee of one hundred dollars. B. A lessor of rental motor vehicles with one or more locations within the state will not be required to file more than one notice. Acts 1989, No. 773, §2, eff.
