RS 40:39 - Issuance of short-form birth certification cards A. The state registrar shall issue short form birth certification cards containing a true certification of name and birth facts as recorded on the original documents on file in the vital records registry, and for use as legal proof of the recorded facts of birth for all purposes. B. Short-form birth certification cards and abstracts shall be issued only for those births registered with the central vital records registry within twelve years of the date of birth. C. An applicant requesting the issuance of a 'long-form' certified photocopy or certified record of the original birth record in lieu of the 'short-form birth certificate card' may obtain it by submitting an application and appropriate fee.
