(a) An application for license renewal shall be accompanied by: (1) a statement of the hours of operation of the licensed premises; (2) a statement of the name of the manager of the licensed premises; and (3) a copy of the current statement for the licensed business that shows that all applicable taxes are paid. (b) In addition to the statements required under subsection (a) of this section, an application for renewal of a Class B license shall be accompanied by a statement of the average monthly sales of food and alcoholic beverages for the licensed premises.
