Section 17-318 - Change of Name of Licensee or Firm

(a) To change the name of a licensee or a firm on a license certificate and pocket card, a licensee shall submit to the Commission: (1) an application on the form that the Commission provides; (2) the license certificate and pocket card of the licensee; (3) any documentation about the name change that the Commission requires; and (4) a fee set by the Commission. (b) On receipt of the application, fee, and any required documentation, the Commission shall issue, to the licensee, a new license certificate and pocket card containing the new name of the licensee or firm.
