In addition to any other information required under this subtitle, any notice given by a museum under this subtitle shall contain: (1) If known, the name of the lender or the designated agent of the lender; (2) If known, the last known address of the lender or the designated agent of the lender; (3) A brief description of the property on loan; (4) If known, the date of the loan; (5) The name of the museum; and (6) The name, address, and telephone number of the person or office at the museum to contact regarding the property.
