§ 9-5-406

(a) An applicant for a license shall submit an application to the Department on the form that the State Superintendent requires. (b) An application for a license shall contain: (1) The name of the applicant; (2) The proposed location of the child care center; (3) The name of the individual to be in charge of the child care center; and (4) Any other information that the Department requires.
