(a) An applicant for a license shall submit an application to the Secretary. (b) The application shall: (1) Be on a form and accompanied by any supporting information that the Secretary requires, including documentation that the Maryland Health Care Commission has determined that the freestanding ambulatory care facility either received a certificate of need or is exempt from certificate of need requirements; and (2) Be signed and verified by the applicant.
