(a) To apply for a pharmacy permit, an applicant shall: (1) Submit an application to the Board on the form that the Board requires; and (2) Pay to the Board an application fee set by the Board. (b) For each pharmacy permit for which a person applies, the person shall submit a separate application and pay a separate application fee.
