Section 12-608 - Application

(a) An applicant for a certificate shall: (1) submit to the State Fire Marshal an application on the form the State Fire Marshal provides; (2) submit all documents that the State Fire Marshal requires; and (3) pay to the State Fire Marshal an application fee of $100. (b) The application form provided by the State Fire Marshal shall contain a statement advising the applicant that willfully making a false statement on an application is a misdemeanor, subject to a fine or imprisonment or both, as provided in § 12–616 of this subtitle.
