The county clerk of each county that alone constitutes 1 or more senatorial or representative districts shall, on suitable blank forms furnished by the secretary of state, transmit without delay to the secretary of state a copy of the certificate of determination certified by the county clerk under the county clerk's hand and seal of office. The secretary of state shall specify to the county clerk whether the certificate of determination must be transmitted to the secretary of state electronically or by mail. The county clerk shall at the same time report to the secretary of state the post office address of each individual elected in the county to any county office or to the office of state senator or representative in the legislature.
