62J.052 PROVIDER COST DISCLOSURE. Subdivision 1. [Repealed, 2007 c 147 art 15 s 22] Subd. 2. Pharmacies. (a) Each pharmacy, as defined in section 151.01, subdivision 2, shall provide the following information to a patient upon request: (1) the pharmacy's own usual and customary price for a prescription drug; (2) a record, including all transactions on record with the pharmacy both past and present, of all co-payments and other cost-sharing paid to the pharmacy by the patient for up to two years; and (3) the total amount of all co-payments and other cost-sharing paid to the pharmacy by the patient over the previous two years.
