Applications for licenses under this chapter are to be made on forms prescribed by the commissioner for each separate warehouse or, if an applicant owns more than one (1) warehouse at any one (1) location, which does not exceed eight (8) miles in distance, then all the warehouses at that location may be included in one (1) application. Every application is to be accompanied by an application fee of one hundred fifty dollars ($150.00) and a certified financial statement in a form prescribed by the commissioner and such further information as the commissioner may by regulation require.Laws, 1977, ch. 409, § 6, eff. 7/1/1977.
