Title XII PUBLIC HEALTH AND WELFARE Chapter 205 • Effective - 28 Aug 1985 205.020. Form of ballot — vote necessary to adopt. — 1. The question shall be submitted in substantially the following form: Shall there be a maximum tax of ______ per hundred dollars assessed valuation for a county health center and the maintenance and operation of same? 2.
