Title XII PUBLIC HEALTH AND WELFARE Chapter 209 • Effective - 28 Aug 1994 209.297. Applications for certification, content, oath — fee not refundable — applicant to be given date for evaluation. — 1. Applications for certification as an interpreter: (1) Shall be submitted in writing to the commission on forms prescribed by the commission and furnished to the applicant; (2) Shall satisfactorily evidence the applicant's education, training, experiences, certification, at the time of application, the applicant is eighteen years of age or older and other information as the commission may require; (3) Shall contain a statement that it is made under oath or affirmation and that the information contained therein is true and correct to the best knowledge and belief of the applicant and that the applicant is subject to the penalties for making a false affidavit or declaration; (4) Shall be accompanied by the required application fee, submitted in a manner as required by the commission and shall not be refundable. 2.
