Portable amusement rides, itinerary to be filed, contents. — The owner or operator of portable amusement rides shall file an itinerary with the department on a department form no less than fifteen days before the operation of an amusement ride for use by the public. The itinerary shall include the following: (1) The name of the amusement ride owner; (2) The carnival, fair, or activity sponsor; (3) The address and telephone number of the site; (4) The dates open to the public; and (5) The name of the contact person on site. -------- (L. 2004 H.B. 1403) Effective 1-01-05
