23-5-705. Application for permit. (1) A nonprofit organization may apply to the department for a casino night permit on a form prescribed and furnished by the department. The application must be accompanied by a fee of $25. (2) The application must include: (a) the name and address of the applicant; (b) the name and address of the applicant's officers; (c) the location, date, and time at which the applicant will conduct the casino night; (d) sufficient evidence concerning the structure and operation of the organization to enable the department to determine whether the applicant is a nonprofit organization; and (e) other relevant information requested by the department.
