37-18-803. Veterinary retail facility -- license required. (1) A person, partnership, association, corporation, or limited liability company may not open, establish, operate, maintain, or do business as a veterinary retail facility without first obtaining a license from the board of pharmacy. (2) The board of pharmacy shall prescribe the license application form. (3) An application or renewal application must be accompanied by a fee set by board rule. (4) A license must be renewed annually. (5) Separate applications and separate permits are required for each veterinary retail facility opened, established, operated, or maintained by the same owner and for a change of location, name, or ownership of an existing veterinary retail facility.
